Culvert Replacement

HOYT RD. AND LORDS MILL RD. CULVERT REPLACEMENT - SEALED BID PROJECT

SCOPE OF WORK: LORDS MILL ROAD

Install 80 feet of 4 foot culvert (supplied at job site)

Bottom of culvert is approx. 15 feet from road surface.

Remove old metal culvert and bring to Epsom Highway Dept.

All aggregate materials will be picked up at Epsom Highway Dept.

Concrete headwalls (supplied at job site) will replace the existing rock headwall

Contractor will use existing material to backfill to subgrade

Pavement will be saw cut and emulsion used when repaving trench.

Contractor is responsible for all cleanup when completed.

Silt fence will be removed by the Epsom Highway dept.

Contractor will follow N.H. DOT Best Management procedures.

All signage and traffic control will be supplied by the Epsom Highway Dept.

Culvert replacement will start no sooner than August 1st and be completed no later than September 30th.

SCOPE OF WORK: HOYT ROAD

Same as above accept the bottom of the culvert is approx. 7 feet from road surface.

ADDITIONAL REQUIREMENTS: The Contractor agrees that it will carry any and all insurance which will protect it, the Town of Epsom and their officials, agents, volunteers and employees from any and all claims and demands, costs, damages, loss of service or consortium, expenses, compensation, and attorneys’ fees including but not limited to any and all claims for personal injury and/or death, workers’ compensation injuries, and property damages which may, in any way, arise from or out of the operations of the Contractor whether such operations be performed by the Contractor itself, anyone directly or indirectly employed by it or any other persons or company retained in any way to carry on all or portion of the operations, activities or work required by or related to the Contract.  The Contractor further agrees that the Town of Epsom and its officials, agents, volunteers and employees shall be named as an additional insured in any and all such liability insurance policies required by the Town of Epsom.

SITE INSPECTION: There will be a site inspection meeting on April 29, 2019 at 9:00 a.m. Meet at the corner of Lords Mill Road and Hoyt Road Epsom NH.

BID DELIVERY: Sealed bids must be delivered by May 6, 2019 at 5:00 p.m. to
